- Format
- The real NCLEX-RN is a computer adaptive test (CAT) of 85 to 150 items with a five-hour limit that includes breaks. At the minimum length, 52 items come from the eight Client Needs content areas, 18 items form three clinical judgment case studies, and 15 are unscored pretest items. Items include multiple choice, multiple response and other Next Generation NCLEX formats. This practice mock has 85 four-option multiple-choice questions spread over the Client Needs categories in proportion to the 2026 test plan (effective April 1, 2026). It does not include case-study or select-all item formats.
- Passing score
- The NCLEX-RN has no percentage pass mark. The adaptive test decides pass or fail by comparing your ability with the passing standard set by NCSBN. This site uses 70% correct as a practice goal only; it is not an official score. There is no minimum for each category on this site.
- Eligibility
- You apply for licensure to your state or territory nursing regulatory body, which decides your eligibility (usually graduation from an approved registered nursing program). You also register with Pearson VUE. You can take the exam only after the board makes you eligible and you receive an Authorization to Test.

NCLEX-RN study tips
Written by our team- Study order
- Begin with Management of Care and Safety and Infection Prevention and Control, which together make up about a quarter to over a third of the test plan and teach the priority and delegation rules used everywhere else. Then study Physiological Integrity, the largest group on this site's mock (43 of 85 questions), starting with Pharmacological and Parenteral Therapies. Finish with Health Promotion and Maintenance and Psychosocial Integrity.
- Topics people often miss
- Pharmacological and Parenteral Therapies mixes drug effects, side effects, dosage calculation and IV care, so it is easy to lose points there. Reduction of Risk Potential needs normal lab ranges and early signs of complications, and Physiological Adaptation needs quick recognition of emergencies such as shock, sepsis and dangerous heart rhythms. In Management of Care, delegation questions are tricky: remember what can and cannot be delegated to assistive personnel and LPN/LVNs.
- Test day tips
- The real exam can be 85 to 150 items in five hours including breaks, so keep a steady pace and do not rush the first questions. Read the last sentence of each question first to see what it asks (first action, need for follow-up, or correct teaching). When two answers look right, choose the one that addresses safety and the most unstable problem first.

NCLEX-RN exam trends
According to the 2026 NCLEX-RN test plan, the content area percentages are: Management of Care 15-21%, Safety and Infection Prevention and Control 10-16%, Health Promotion and Maintenance 6-12%, Psychosocial Integrity 6-12%, Basic Care and Comfort 6-12%, Pharmacological and Parenteral Therapies 13-19%, Reduction of Risk Potential 9-15%, and Physiological Adaptation 11-17%. Compared with the 2023 plan, the percentages stayed the same and "Safety and Infection Control" was renamed. The split of each category into the chapters on this site is a study grouping, not an official breakdown.
Start with Management of Care and Pharmacological Therapies, the two largest areas. Practice deciding what to do first: airway, breathing and circulation, and the client whose condition is unstable or unexpected. Do a short set every day and review the explanation of every wrong answer.
About NCLEX-RN
The NCLEX-RN is the national licensure exam that registered nurses in the United States must pass to be licensed. It is developed by the National Council of State Boards of Nursing (NCSBN) and delivered by computer at test centers. The 2026 test plan organizes the exam around four Client Needs categories: Safe and Effective Care Environment, Health Promotion and Maintenance, Psychosocial Integrity, and Physiological Integrity.
FAQ
- Q. How many questions are on the NCLEX-RN?
- The NCLEX-RN is adaptive, so the length varies from 85 to 150 items. You have up to five hours, including breaks. Fifteen items are unscored pretest items, but you cannot tell which ones.
- Q. What score do I need to pass?
- There is no fixed percentage. The computer keeps estimating your ability and stops when it is 95% certain you are above or below the passing standard, or when you reach the maximum length or run out of time. The 70% goal on this site is only for practice.
- Q. Are these real NCLEX questions?
- No. NCLEX items are confidential. Every question on this site is an original practice question written with AI to match the published test plan, then checked by a separate review that solved each question again. Mistakes are still possible; please use your nursing textbooks and course materials as your main reference.
- Q. Does this site include Next Generation NCLEX case studies?
- Not yet. The practice questions here are four-option multiple choice. The real exam also includes clinical judgment case studies and other item types such as multiple response, drag-and-drop and highlight items.
